Community Care Assessment

The Community Care Assessment is a research project that builds on any existing community assessment efforts to better define community partners' missions, priorities, and capabilities. Our Community Care Assessment work maps out the common barriers and challenges members face when navigating their care needs. Plan population health efforts are better informed when a plan understands members' care experience and works to build collaborative relationships with community partners.

The Community Care Assessment work helps plans understand community partners' capabilities, how their workforce can support a plan's efforts, and what opportunities exist to deliver a more integrated care experience for members. 

What's Involved

This short-term project is led by a multidisciplinary team of public health researchers, project specialists, and data analysts. It leverages local partnerships with community members, Federally Qualified Health Centers, faith-based organizations, community centers, community-based organizations, and academic institutions.

A local Community Advisory Council of key stakeholders in the fields of health care, research, and social services is formed at the start of the project and continues to guide the project and dissemination of findings. 

The work begins with a review of the secondary data sets on health and Social Determinants of Health (SDOH) of the local communities as well as a review of previously published health assessments or reports.

Focus Groups

Community members are recruited from local health clinics, faith-based organizations, and community-based organizations to participate in focus groups to evaluate components of the community care model such as:

   • Social Determinants of Health (SDOH) assessment
   • Plan care system
   • Technology
   • Delivery method

The findings are co-analyzed, and recommendations are co-developed with community stakeholders and help our partners better understand the impact of community factors on health and wellbeing, including gaps in services and barriers and facilitators to care in the different counties. Through strengths-based and solutions-focused approaches, this work can help guide new programs and initiatives to promote community-driven solutions to health equity for these communities and support a more tailored approach to the local Path Assist delivery.
